As for Elementary - I've seen a lot of people suggest hallucination and was kind of confused the first time? Because to me it seemed like a) Joan would be more concerned with Sherlock freaking out than with this woman she's never met before (until the last second when she realizes "Oh, Irene!" and stares at her) b) the opera music was sufficiently loud to keep Irene from hearing Joan's quiet "Sherlock?"
But then again, I've assumed since we learned of Irene's death that it had been faked. So I clearly went into this with a different perspective from most.
